Enroll a Child



Listed below are the steps to enroll a child in St. Mary’s Child Center.

  1. Fill out the Initial Application below or by calling 317-361-4893.
  2. You will then receive a phone call to set up an appointment to fill out additional forms.

You will need to bring the following items with you:

  • Birth Certificate
  • Immunization Records
  • Food Stamp Case Number (if applicable)
  • Pay Stub and/or School Enrollment Verification (if applicable)

 

  • Your child will then be placed on the waiting list and as soon as an appropriate opening occurs, you will be called to verify placement.
