Enroll a Child



Listed below are the steps to enroll a child in St. Mary’s Child Center.

  1. Fill out the Initial Application below or by calling 317-361-4887.
  2. You will then receive a phone call to set up an appointment to fill out additional forms.

You will need to bring the following items with you:

  • Birth Certificate
  • Immunization Records
  • Food Stamp Case Number (if applicable)
  • Pay Stub and/or School Enrollment Verification (if applicable)

3. Your child will then be placed on the waiting list and as soon as an appropriate opening occurs, you will be called to verify placement.
